Position Summary.
Reporting to the Chief Pharmacist, the incumbent will ensure correct interpretation of prescriptions and physician orders at both in-patient and outpatient levels to enable accurate compounding, dispensing, patent education and handling of pharmaceutical supplies for all orders.
Key Responsibilities.
• Review & verify written prescriptions for authenticity, validity, incitation, dose, strength route. Form. duration, drug interactions and assure that drugs are dispensed in proper manner.
• Prepare. compound and dispense drugs and other pharmaceutical supplies.
• Identify, resolve and prevent potential/actual drug-related problems and reviews patient profiles for drug interactions, allergies, contraindications and adverse reactions to determine and Implement optimal drug therapy.
• Conduct patient counselling on medicines. devices and adherence to therapy.
• Ensure proper inventory management of drugs and supplies, their storage, handling and updating of the inventory records on a regular basis.
• Monitor drug consumption and prepare reports regarding drug usage in the hospital.
• Maintain strict control over usage of controlled drugs such as narcotics for compliance with policy on dangerous drugs.
• Facilitate nursing floor Inspections on a monthly basis and satellite inspections daily.
• Maintain accurate documentation of all interventions and recommendations in regards to drug therapy and rational use of drugs, and their outcomes.
• Monitor and record adverse drug reactions and medication incidents
Requirements
Applicants for the above position must have a Bachelors of Science Degree in Pharmacy and a Practicing License from the Pharmacy and Poisons Board of Kenya. S/he should be registered with Pharmaceutical Society et Kenya and have a minimum of 1-year hospital experience working in pharmaceutical services. Clinical pharmacy experience and evidence of continuous education will be an added advantage.
